ATTENTION: Prescribers may notice receipt of emails from the CSMD that identify potential high risk patients associated with their current practice. These will be displayed on your Clinical Notification panel on your home page.
To view Clinical Notifications associated with your account, click “View all…” from the Clinical Notification panel on your account Home Page. This is a historical view and will always display the last five weeks of notifications. Please note the columns are sortable to assist in reviewing. To view the patient’s CSMD report, click on the icon along the same row for each respective patient. Please keep in mind, there is no prescribed action required on your part, the Clinical Notifications are for informational purposes.
Important: The CSMD Program requires each user to have a unique private and confidential email address. Also, CSMD anticipates migrating to a new platform in the near future and successful migration requires each user to have a unique private and confidential email address. Review your email address by going to “My Account” after you login to ensure that your account has a unique private and confidential email that only you can access to ensure compliance and successful migration. While on the “MY ACCOUNT” page, review your other information as well as your security questions and make any necessary updates. Please be sure to periodically review your information and update as necessary and always click “Save” after making any updates.
Browser compatibility: The supported browsers that provide the best performance of the CSMD include Internet Explorer 8 or above, Safari, Chrome, and Firefox. At this time Edge is incompatible with CSMD.
When registering the information you provide must match the information as it appears on your Board License. If you experience issues logging into the system (EX: invalid Board License, invalid DEA) please contact the CSMD Admin via email donotreply@appriTest.com or phone 615-253-1305.
